Is Liposuction Surgery Right for you

You may be a candidate for liposuction if you have excess subcutaneous fat, which is resistant to diet and exercise. Liposuction may also be desirable if you wish to change the contours of your body. Commonly involved areas are the anterior part of the neck (under the chin), upper arms and armpits, male chest/breasts, trunk, abdomen, waist, hips, thighs, inner knees, calves, and ankles. Most patients are near normal weight and desire permanent contour changes.

Improved techniques now allow larger amounts of fat removal. Liposculpture can be useful to heavier persons as well.

Liposuction is intended to produce a permanent improvement in the contour and proportion of the treated areas. A more flattering figure or physique, in or out of clothing, is another goal of liposculpture.

Liposuction is done on an outpatient basis. General anesthesia is usually used for safety and comfort. Small suction tubes (cannulas) are inserted through very short incisions placed in inconspicuous locations near the fat deposits, and excess fat is removed. If large volumes of fat are to be removed, we will probably ask you to donate one or two units of blood and take supplemental iron before surgery. Your own blood will be used as a transfusion at the end of surgery.

The patient goes home with a compression garment (or tape) covering the treated areas. Compression is advised for about a month as it helps reduce swelling after liposuction. Once pain and soreness decrease, it is frequently more comfortable to wear lycra or spandex exercise pants. Initial discomfort is easily controlled with oral medication. Bruising and swelling usually subside in 2-4 weeks. The final result takes shape over 2-3 months.

Additional procedures that may enhance the result of liposuction are Tummy Tuck (Abdominoplasty), Breast Reduction, Thigh Lift, or Buttock Lift. Liposuction is considered cosmetic and therefore is not covered by insurance. The patient is responsible for payment

Introduction

Maintaining a rejuvenated face is todays social requirement and more so for personal psychological relief. The nagging though of not being good looking or the thought of being old is disabling, and particularly when today we can modify a person’s looks by the skillful use of Plastic Surgery. Facial Lipostructure

Dr Goldmann is famous for his work on fat injections and using fat for facial rejuvenation. The area of the face which is most responsive to fat injection are the nasolabial lines. Use of restylane and other fillers is common in the nasolabial lines, the naso-jugal groove, and other areas showing depression. But the problem with fillers is that they are expensive and are temporary. The latter aspect is also thought to be a beneficial aspect, since any irregularity is at most going to last for 5-6 months. However here fat scores very highly, in that the procedure is a one time affair, uses self fat (autologous fat transfer) and tends to be permanent. It is specially suitable for the nasolabial lines, which are notorious for their irresponsiveness to treatments. Special instruments are available for releasing the nasolabial  lines and then injecting fat there to correct the depression.

 

The trick in autologous fat transfer is the processing of fat. It is important to not to expose the fat to air, since it degenerates very fast. Also we must be careful to isolate the fat cells, and not the oil. Further there is a distinct stem cell layer in the form of a clot. It is mandatory to preserve this. Failure to follow these precautions is responsible for the oft repeated poor results seen with fat injections. Fat has to be treated gently like babies.

 

Lip Augmentation

Many men and women are born with thin lips, and there is a constant desire to achieve more fullness in this region. Thus a number of clients request for filler injections in the lip and along the lip margin. Again we need to remember, that fat is an excellent filler for lip augmentation. Fillers are better to highlight the lip margins, because the size of fat cells tends to leave a clumpy appearance at the lip margins.

 

Possible problems with autologous fat

 

Injection of fat necessitates a small liposuction in the region of the abdomen and/ or thighs to harvest fat. Though this can be done under local anesthesia, the patient must know that an additional procedure is required, since fillers come prefilled into syringes. The harvested fat is then carefully processed and transferred to smaller syringes. This is then injected into the desired area. There is a swelling in the injected region. In the region of the lip, this can last for between 3-6 weeks, so social appearance in this time will need to be curtailed.

30-50% of the injected fat may be resorbed, and thus whenever we inject fat, we always inject about 30% extra. This is responsible for the over filled look initially but always settles down within 6 weeks.

Fat removal is accomplished as the suction cannula creates tiny tunnels through the fatty layers. After surgery, these tiny tunnels collapse resulting in an improved body contour.

After liposuction, the new body’s shape is more or less permanent. If a patient does gain a moderate amount of weight after liposuction, then the figure will simply be a larger version of the new body shape. Contrary to popular belief, fat cells that are removed by liposuction do not grow back. Liposuction removes fat cells permanently. With a small weight gain, existing fat cells simply get bigger by accumulating more fat within the existing cell. If the patient does not gain excessive amounts of weight, then the new more pleasing silhouette is permanent. Of course after liposuction, the clock keeps ticking, and advancing age will produce the usual changes in the shape of the body associated with the aging process. Nevertheless, the benefits of liposuction will always be apparent.

Patients should not expect to lose a dramatic amount of weight with liposuction. However, because fat is removed from cosmetically important areas, liposuction should produce significant improvements in aesthetic appearance. Although liposuction should not be regarded as a method for weight loss, in appropriate patients it can produce significant cosmetic improvements.

Most patients will see 90% of their ultimate liposuction results with in one to three months after surgery. For the first few weeks after surgery there is postoperative swelling. The rate at which this swelling subsides is highly variable from patient to patient.

The procedure can be done on an out-patient basis or as a hospital admission depending on the amount of fat to be removed. This is usually done under sedation or general anesthesia depending on the extent of surgery. Bruising and swelling are expected post-operatively. This usually subsides after several weeks.

So what exactly is buttock augmentation? It is a cosmetic surgery procedure involves lifting the buttocks area and enlarging them through the use of a synthetic implant or silicone.

Buttocks implants are more “hardy” than breast implants. They are made of an elastomer which does not carry the chance of leakage. The procedure is done by making a small incision above the muscles in the sitting area and below the fascia. Consider power assisted liposuction – The MicroAire PAL uses a 2-mm reciprocating movement at 4000 cycles per minute to facilitate the procedure. This means that less force is required, especially in areas with more fibrous tissue. That makes the lipoplasty procedure easier on patients. The PAL is not ultrasonic.
